Hey moms and dads. We want to make sure your kids are safe and protected when they're participating with our youth group. If you haven't done so already, please take a moment to click on the link below for our Parental Consent and Release Forms. Filling them out completely and accurately will ensure that we can obtain medical care in the unlikely event of an accident or emergency. Simply print out all three pages, complete the forms, and return them to Scott Bomar, Minister with Students, to cover your child for the 2007-2008 program year.
